What is Care Navigation?
Care Navigation is a system designed to help patients access the most appropriate service or healthcare professional for their needs. When you contact the practice, our trained team will ask you a few questions to ensure you are directed to the right person or service as quickly and efficiently as possible.
Why Do We Use Care Navigation?
Healthcare services are evolving, and many concerns or conditions can now be addressed by specialists outside of the GP. By guiding you to the right professional, we can:
- Ensure quicker access to care.
- Reduce unnecessary delays.
- Make the best use of available resources to serve all patients effectively.
Examples of Services You May Be Directed To:
- Nurse or Healthcare Assistant: For blood tests, vaccinations, or routine health checks.
- First Contact Physiotherapist: For musculoskeletal concerns like back pain or joint issues.
- Social Prescriber: For help with non-medical issues such as loneliness, housing, or financial concerns.
- Mental Health Practitioner: For support with low mood, anxiety, or other mental health concerns.
- Pharmacy Team: For medication reviews, minor ailments, or prescription queries.
How It Works:
- You contact us via phone, online or at reception.
- Our trained reception team (also known as Care Navigators) will ask you a few brief questions about your issue.
- You are guided to the most appropriate service or clinician, which may include seeing a GP if necessary.
